If THREE WEEKS have passed, and you haven't received your order:

The name under which the merchandise was ordered (that could be you, or, if you used the name/e-mail account of a friend or relative, use that name/e-mail account when requesting a follow-up), the date you placed the order, the items that you ordered, and the correct shipping address.

The Lackey will get back to you to confirm the status of your goods.

If you MADE A MISTAKE in your order:

If you made a mistake when you placed your order (need a different size, ordered the wrong item) then fill out this form. The Lackey will check on your order and e-mail a reply. You must include this information to expedite the Lackey's search:

The name under which the merchandise was ordered (that could be you, or, if you used the name/e-mail account of a friend or relative, use that name/e-mail account when requesting a follow-up), the date you placed the order, the items that you ordered, the items that you really want, and the correct shipping address.

If your order arrived INCOMPLETE OR INCORRECT:

If your order has arrived, but was incomplete or incorrect then the Lackey needs to correct the mistake right away. First please accept an apology from the Lackey, then let us know by filling out this form. You must include this information to expedite the Lackey's mea culpa:

The name under which the merchandise was ordered (that could be you, or, if you used the name/e-mail account of a friend or relative, use that name/e-mail account when requesting a follow-up), the date you placed the order, what is wrong with the order, and the address where any further merchandise should be sent.
